what can i do to prove a disability when I go to the state examiner.?

I suffer from cervical spinal stenosis with c5/c6 nerve impingement, I have extevsive damage in both shoulders, I suffer from severe depression as a result frommy condition. How can I prove depression when I go to the state examiner?

C-Spine MRI Reveals Following (Please Help Interpret):?

FINDINGS: There is retrolisthesis of C2 on C3 and C3 on C4. There is mild reversal of the normal curvature of the cervical spine. There is disc dissication noted involving all of the cervical disc levels.
There are posterior disc/osteophyte ridge complexes noted at the C3-4, C4-5, and C5-6 levels, impinging upon the anterior thecal sac. [...]

Central and left sided posterior protrusion of the c4-c5 disc is present indenting the spinal cord slightly along its anterior aspect. There is no high grade stenosis or cord compression. MR THORACIC SPINE
Histroy - NF, back pain.
Technique: MR of the thoracic spine was performed using sagittal T2/STIR/pre and postcontrast T! and axial T2/ pre and post contrast T1- weighted imaging.
Findings:
There is server levosoliosis of the upper thoracic spine and moderate dextroscoliosis of the lower throacic spine. Harrington rods are seen in the paraspinal region. [...]
